How does the Penile Implant Work?
There are three types of penile implants: 1) three-piece inflatable implants, 2) two-piece inflatable implants, and 3) malleable implants. The largest manufacturers of penile implants are Boston Scientific and Coloplast. Dr. Green is very experienced with all three types of implants, and works closely with both Boston Scientific and Coloplast to stay current with the most recent research.
Three-Piece Inflatable Penile Implant
The three-piece inflatable penile implant is the gold standard for penile implants as it is the most natural-feeling and expansive implant on the market.
This type of implant is called a three-piece device because it consists of three components: two inflatable cylinders implanted into the penis, a pump implanted in the scrotum, and a reservoir implanted in the abdomen. The three components require only one incision to be inserted.
To create an erection, the man squeezes a button hidden in the scrotum that pumps a saline fluid from the reservoir (tucked into the abdomen) into the cylinders (in the penis) causing the cylinders to inflate and expand. This creates a rigid erection that can be maintained for as long as desired. To deflate the implant, the man simply presses a release valve (also hidden in the scrotum), allowing the fluid to return to the reservoir and the cylinders to become soft and flaccid. This penile implant is a closed system and never requires maintenance or refilling of fluid.
A three-piece penile implant allows for a more natural-looking and feeling erection than a malleable implant. Because the pump and reservoir are contained within the body, there is no visible evidence of the implant.
Two-Piece Inflatable Penile Implant
Like the three-piece implant, the two-piece implant consists of inflatable cylinders that are implanted into the penis and a pump that is implanted in the scrotum. However, the two-piece implant does not include a separate reservoir, which is instead incorporated into the pump itself. As there is no reservoir involved, this device is normally reserved for those with extensive abdominal surgery or scarring. Just like the three-piece implant, the two-piece works by squeezing the pump hidden in the scrotum to move fluid into the cylinders in the penis to achieve an erection. Unlike the three-piece implant, the device is deflated by physically bending and manipulating the cylinders to move fluid back into the pump in the scrotum.
Malleable (Semi-Rigid) Penile Implant
Unlike inflatable penile implants, malleable implants do not require a pump or reservoir to function. A malleable penile implant is comprised of two bendable rods that are placed within the erectile bodies of the penis. These rods can be flexed into an erect position to allow men to participate in intercourse. They can then be bent back down to a more concealed state when an erection is not needed. These devices can be a great option for men with limited dexterity or men who lack the ability to manipulate the pump of an inflatable penile implant.

The procedure is a short day surgery that takes about 45 minutes in the operating room. You will be asleep for the procedure under general anesthesia and will be able to go home the very same day. The entire procedure is performed through a small, 4 cm, incision between the penis and the scrotum. Through this small incision we are able to place all components of the penile implant.
After the procedure there will be a supportive dressing over the incision that will stay on for 48 hours. After 48 hours, you may remove the dressing and shower allowing soap and water to run over the incision. Most people are sore for the first 1-2 weeks and are able to return back to light duty around that time. Full duty including swimming and heavy lifting can be resumed 4-6 weeks after the surgery.
The implant should not be manipulated for 4-6 weeks after the procedure. At 4-6 weeks, Dr. Green will see you in his office and teach you how to use the device.
